Patients with Chronic Kidney Disease Do Not Need to Be Treated with Vitamin D Since They Cannot Activate It It is true that the kidneys are the major source for the production of 1,25(OH)2 D that acts as an endocrine hormone to regulate calcium and bone metabolism. As a result, patients with chronic kidney disease (CKD) often require either 1,25(OH)2 D or one of its active analogues to help prevent an increase in their PTH levels and associated metabolic bone disease (4, 76). However, the National Kidney Foundation recommendation known as the KDOQI guidelines recommends that all patients with CKD maintain a blood level of 25(OH)D of >30 ng/ml.

Above the age of 65, there is fourfold decrease in the capacity of the skin to produce vitamin D3 when compared with a younger adult (13). An alteration in the zenith angle of the sun caused by a change in latitude, season of the year, or time of day can dramatically influence the skin’s production of vitamin D (4, 5, 14). Above and below latitudes of approximately 35◦ north and south, respectively, vitamin D synthesis in the skin is absent during most of the winter. In nature, very few foods contain vitamin D (Table 1).

The reason for this is that it is now recognized that the parathyroid glands have the ability to produce locally 1,25(OH)2 D which may help suppress the overproduction of PTH (78). Furthermore, the 25(OH)D may be also converted to 1,25(OH)2 D locally within noncalcemic tissues such as colon, prostate, breast which may help regulate cell growth and prevent malignancy (4, 5). 4. , 50,000 IU of vitamin D2 once a week for 8 weeks. , a 25(OH)D <20 ng/ml and for obese patients, I usually extend the treatment for an additional 4–8 weeks.
